Medication-Facilitated Advanced Airway Management with First-Line Use of a Supraglottic Device – A One-Year Quality Assurance Review

Introduction:
Airway management is a controversial topic in modern Emergency Medical Services (EMS) systems. Among many concerns regarding endotracheal intubation (ETI), unrecognized esophageal intubation and observations of unfavorable neurologic outcomes in some studies raise the question of whether alternative airway techniques should be first-line in EMS airway management protocols. Supraglottic airway devices (SADs) are simpler to use, provide reliable oxygenation and ventilation, and may thus be an alternative first-line airway device for paramedics. In 2019, Alachua County Fire Rescue (ACFR; Alachua, Florida USA) introduced a novel protocol for advanced airway management emphasizing first-line use of a second-generation SAD (i-gel) for patients requiring medication-facilitated airway management (referred to as “rapid sequence airway” [RSA] protocol).
Study Objective:This was a one-year quality assurance review of care provided under the RSA protocol looking at compliance and first-pass success rate of first-line SAD use.
Methods:Records were obtained from the agency’s electronic medical record (EMR), searching for the use of the RSA protocol, advanced airway devices, or either ketamine or rocuronium. If available, hospital follow-up data regarding patient condition and emergency department (ED) airway exchange were obtained.
Results:During the first year, 33 advanced airway attempts were made under the protocol by 23 paramedics. Overall, compliance with the airway device sequence as specified in the protocol was 72.7%. When ETI was non-compliantly used as first-line airway device, the first-pass success rate was 44.4% compared to 87.5% with adherence to first-line SAD use. All prehospital SADs were exchanged in the ED in a delayed fashion and almost exclusively per physician preference alone. In no case was the SAD exchanged for suspected dislodgement evidenced by lack of capnography.
Conclusion:First-line use of a SAD was associated with a high first-pass attempt success rate in a real-life cohort of prehospital advanced airway encounters. No SAD required emergent exchange upon hospital arrival.

Previous studies of depression after stroke have reported widely variable findings, possibly due to differences between studies in patient characteristics and methods for the assessment of depression, small sample sizes, and the failure to examine stroke-free reference groups to determine the base rate of depression in the general population. In an effort to address certain of those methodologic issues and further investigate the frequency and clinical determinants of depression after stroke, we administered the Structured Interview Guide for the Hamilton Depression Rating Scale (SIGH–D) and neurological, neuropsychological, and functional assessments to 421 patients (age = 71.5 ± 8.0 years) 3 months after ischemic stroke and 249 stroke-free control subjects (age = 70.8 ± 6.7 years). We required a SIGH–D total score > 11 for the identification of depression. We found that depression was less frequent (47/421 patients, or 11.2%, and 13/249 control subjects, or 5.2%), less severe, and less persistent in our stroke cohort than previously reported, possibly due to the underrepresentation of patients with a premorbid history of affective illness. Depression was associated with more severe stroke, particularly in vascular territories that supply limbic structures; dementia; and female sex. SIGH–D item analyses suggested that a reliance on nonsomatic rather than somatic symptoms would result in the most accurate diagnoses of depression after ischemic stroke. (JINS, 2003, 9, 429–439.)

Summary
Introduction
Many studies have investigated focal brain lesions as a basis for cognitive impairment but less attention has been given to their role in the etiology of mood and behavior disorders. Both as a cause and as an effect of that disparity, the measures that are available for the cognitive assessment of patients far outnumber those that might be utilized for the assessment of disorders of mood and behavior. Although there is currently greater potential for the effective pharmacologic treatment of mood and certain behavior disorders than of cognitive impairment in patients with focal brain lesions, it is likely that many patients with such disorders remain undiagnosed due to the underutilization of those standardized assessments that are available and to an understandable emphasis on the physical disabilities that may result from those lesions. Thus, this chapter describes certain of the methods that are available for the evaluation of mood and behavior disorders in patients with focal brain lesions as well as some of the difficulties inherent in the evaluation of neurologic patients with those disorders.
Overview of the characteristics of assessment tools Although the diagnosis of major depression has frequently been based on simple clinical judgment in investigational studies, particularly those in which mood disorders were not the primary focus, scales that have been utilized to assist in that diagnosis have taken many forms, varying with regard to the following characteristics:
Mode of administration, i.e., is the scale administered to the patient by an examiner or is it self-administered?
Depth of inquiry regarding specific aspects of the syndrome of major depression, such as vegetative signs.
Duration of symptoms, i.e., must symptoms have been present during a specified period preceding the assessment?
Item scoring method, e.g., true-false, severity ratings.
Potential for use as a formal diagnostic tool, i.e., adherence to codified diagnostic paradigms such as the criteria presented in the Diagnostic and Statistical Manual of Mental Disorders,fourth edition (DSM-IV; American Psychiatric Association, 1994) versus the use of a simple cut-off score or no formal diagnostic method.
The complexity of the scale and the cognitive demands that it places on the patient.
